To get started as a new client, please email me at [email protected].  I will respond with detailed info and links to set up your appointments. If you need to reach me by phone, it is best to email me at [email protected] and I can set up a time for a quick chat.  I do not have a receptionist and can not pick up phone calls very easily.

Referrals: In the State of Illinois, you no longer need a prescription in order to receive treatment but to ensure the best possible insurance reimbursement, but if you have one please email it to me or bring it with you when you come for your first visit.

Insurance: I am an “out of network” practitioner with all insurance companies including Medicare.  Payment is due on the day of service.  Your invoice, or “Superbill”, with all the appropriate codes will always be available in your portal so that you can download and submit to your insurance company.  If you have out-of-network benefits and you have met your deductible, your insurance company will reimburse you directly. Medicare clients are also required to pay on the day of service, but I will submit a CMS-1500 form on your behalf showing that you paid.  They will reimburse you and forward your claim to your secondary insurance. Prior to your first visit, please call your insurance company and inquire about your physical therapy “out of network” benefits.

Once you have set up your patient portal and appointment you will see this: In your confirmation emails you see instructions for entering our secure building and a door code to enter my suite- please keep this information handy! Once you set up your portal, you can schedule and reschedule your appointments, view invoices, enter a form of payment, retrieve your home program instructions, and download your superbill so that you can submit to your insurance company for reimbursement. You will be asked to electronically sign and submit two forms (three signatures total)- these are the Consent to treat and HIPAA form and a COVID-10 policies form- if you don’t receive a prompt to do this, they can be found in “Documents” section of your portal.

There is a PTeverywhere app that you can download if you’d like- it is very easy to use and your exercises will be readily available to you.

FEES

One-hour evaluation is $175.

One-hour in-person or virtual treatments are $160.

Please note that we keep our fees well below the going rates to accommodate our self-pay and out-of-network clients.  Payment is due on the day of service and your “superbill” will be available in your portal at any time for you to submit for reimbursement.  Your insurance company will reimburse you directly based on your plan’s out-of-network benefits.

FAQs

Is a physician’s referral required?

  • In the state of Illinois, a physician’s prescription for physical therapy treatment is no longer required- but you may want one on file to maximize reimbursement. This may be faxed to (773) 755-9216, you may also email it or bring it in with you to be scanned.

How frequently will I need physical therapy?

  • Many chronic conditions are treated most efficiently with weekly one-hour-long sessions.  Acute injuries may require just a few visits, whereas long-standing conditions may require treatment over a longer period of time with a decreasing frequency of visits as you become more independent with managing your recovery.

Is physical therapy covered by insurance?

  • Most insurance plans cover physical therapy.  We request that you call your insurance company and inquire about your benefits.  You will want to know what your “out of network” deductible, co-pay or co-insurance is, as well as any limits to your benefits.  We do not pre-screen your benefits and you will be responsible for paying the balance each session.
What about Medicare?  
  • Lakeview Physical Therapy, P.C. is a non-participating Medicare provider.  Payment is due at the time of service and we will submit the paid claim to Medicare on your behalf.  Medicare will reimburse you directly.
